Education Transcript Request Form
A current or former student may
secure a transcript of his grades
by completing a Transcript Release
Form and submitting it to the
Continuing Education Office either
by mail or fax. In compliance
with the Privacy Act, student
transcripts will not be released
to a third party without the signature
of the student. |

Request for a Duplicate GED
Diploma or Official GED Transcript
Any student who received a GED in North
Carolina can receive a duplicate GED
Diploma or Official GED Transcript by
submitting a Request Form to NC State
GED Office by fax or mail |

Continuing Education Concurrent
Enrollment
Any currently enrolled high school student
at least 16 years of age wishing to
take an applicable occupational Continuing
Education class must have a Concurrent
Enrollment form completed prior to registration.
